I selected Wufoo as our Vancouver twins forms management software. It is much better than Google's forms. It has great data types, easy drag and drop placement, easily customizable form responses including templates!, supports mandatory fields, embeddable links. It supports export to Excel. Just perfect.
It does have a support for Paypal which is great, but it's way too expensive at $24/month.
